Caroline Slezak Kawa was a Canadian woman born on April 30, 1986. She went on to become the lead vocalist of the Canadian alternative rock band Die Mannequin as “Care Failure.” She was based out of Toronto, Canada.
Die Mannequin toured across Canada several times, opening for Buckcherry, Guns N’ Roses, Marilyn Manson, and Sum 41. They have also toured Europe on a number of occasions, alone and as a successful opening act for Danko Jones in 2008.
Apart from being a founding member of Die Mannequin “Care Failur℮” also worked with Big Dirty Band. She also appeared in the movie “Hard Core Logo 2.” Caroline launched her album “Unicorn Streak” in 2007 which is a collection of ten songs by her band.

Caroline “Care Failur℮” Kawa of Die Mannequin died on Thursday, March 30, 2023. She was 36 years at the time of her death. She was about to turn 37 a month later. Unfortunately, destiny had planned something else for the talented vocalist.
The unfortunate news of Die Mannequin’s Care Failure’s death was announced by Nathan Clearihue on Facebook.
“R.I.P Caroline Kawa otherwise know as Care Failure from Die Mannequin. Saw them many times and of course they filmed Hard Core logo 2 here in Regina. She was a wonderful human being. Gone way too soon 🤘🤘,” he wrote.
